I was appoint as a enquiry Officer to conduct an a enquiry on the charges levelled against Mr. ........................ He was charge in following mis-conduct.

Mr. ..................submitted his explanation on ............ not satisfied with the reply and also in order to give him one more opportunity to represent his case, an enquiry was ordered and I was appointed as Enquiry Officer and the enquiry was held on.........a.m. At the Enquiry the management was represented by Shri................Office clerk. The charge sheeted worked did not attend the enquiry. In order to give him one more opportunity, the enquiry was postponed to ... On ....... The enquiry officer suddenly feel sick. Hence the enquiry was again postponed to......... at ......... a.m. At the enquiry, the management was represented by Sri............. Officer Clerk. The charge sheeted worker was represented by Sri...................
Before commencement of the Enquiry, the management’s representative filed the following documents with the consent of the charges sheeted worked which were marked as exhibits A, B, C and D :-

2. Reply of Charge Sheeted - Ex-B
3. Enquiry Notice dated : Ex C
4. One statement containing the details of previous action taken against the charge sheeted worker, he admitted guilty.
The points for determination are :
1. Whether Mr.................had abstained from duty without prior sanction of leave from the competent authority, During the period from ........... to ........... he was absent for more than .........(........) consecutive days.

Before commencement of the Enquiry, the management’s representative filed the following documents with the consent of the charges sheeted worked which were marked as exhibits A, B, C and D :-

4. One statement containing the details of previous action taken against the charge sheeted worker, he admitted guilty.
The points for determination are :
1. Whether Mr.................had abstained from duty without prior sanction of leave from the competent authority, During the period from ........... to ........... he was absent for more than .........(........) consecutive days.
2. Whether his absence from duty from .............. to ............. is a misconduct as defined in clause 20(f) of the A.P. Government Model Standing Orders append to the Industrial Employment ....................Rules.
Point No. 1
In view of the charge sheeted workman’s admission and on the basis of the records submitted by the managements representative as exhibits I have no hesitation to find that Mr........................ was absent from duty for more than ten consecutive days from ........... to ..............
Point No. 2
As per Orders No........ of the Government Orders, absence without leave for more than three consecutive days is a misconduct.
Further as per the records submitted by the Managements representative, as exhibits the charge sheeted workman was already warned thrice for the same charge.
In view of my above findings on points 1 and 2, I find that the charge against Mr........... is proved beyond doubt as misconduct in terms of clause ......... of the Orders appended to the, Industrial Employment .................... Rules.
The connected enquiry papers are enclosed.

In view of the charge sheeted workman’s admission and on the basis of the records submitted by the managements representative as exhibits I have no hesitation to find that Mr........................ was absent from duty for more than ten consecutive days from ........... to ..............
Point No. 2
As per Orders No........ of the Government Orders, absence without leave for more than three consecutive days is a misconduct.
Further as per the records submitted by the Managements representative, as exhibits the charge sheeted workman was already warned thrice for the same charge.
In view of my above findings on points 1 and 2, I find that the charge against Mr........... is proved beyond doubt as misconduct in terms of clause ......... of the Orders appended to the, Industrial Employment .................... Rules.
