Mr......................... is a worker employee in Messers................. By Charge Memo No......... he has charged as follows. On reporting for duty in his section on ......... at.......... (Time). He did not commence his work but deliberately refused to work and willfully persisted in refusing to work and thus failed to carry out work in accordance with the lawful and reasonable orders of the Engineer-in-charge, (i.e.) his superior.
The above act and commission if proved would constitute as misconduct as per standing order No....... of the Model Standing Orders of the Andhra Pradesh Government. The Employee has sent his reply to the show cause notice flatly denying the charge framed against him. An enquiry was ordered. It was held on..........at.............. (Time) at the factory office.
The employee has informed me that did not require the assistance of any of his co-workers in the enquiry.
Before commencement of the enquiry the Management’s Representative. Mr............. has filed with the consent of the workers, the following records which were marked as exhibits, M1, M2, M4 & M5 :-

The contents of the above records were read out and explained to the workers in regional language. When questioned the worker whether he admits the charge, he stated that he admits the charge and prayed for excuse. Since the worker has admitted the charge. I did not proceed further with the enquiry and closed the enquiry.
It is therefore clearly proved as follows :
