By our letter dated ..............charges of willful insubordination and disobedience of the reasonable orders of the superiors, not to go on strike from .......................striking work from.................... and instigating and inciting other co-workers to strike work from ..............on ................and preventing and threatening workers with dire consequences if they enter into the factory, entering company quarter on ............night .... ................... inciting and instigating officers to strike work from ...............and causing disturbance in the company colony during the night of ................. riotous and disorderly behavior in front of the factory premises at about................. on.....................and taking an officer of the company to company quarter .................and instigate him and incite him to strike word, riotous and disorderly behaviors in the colony, creating panicky in the minds of the employees families, thus acting in a manner subversive of discipline have been framed against you, and the charges are being serious, you were suspended from duty with immediate effect by our letter dated ................The charge sheet was sent to you by Regd. post at your last known address and copy exhibited on the factory notice board. We have however not received your explanation so far.

You are hereby given an opportunity to explain, an enquiry is being conducted in the Factory premises................... at ................ (...............) by Shri ...............who has been appointed as the enquiry as the enquiry officer. You may collect a copy of charge sheet if so desired and give your explanation before the Enquiry officer and defend yourself. If you fail to attend enquiry, proceedings, the enquiry will be conducted ex-parte and further action taken without any intimation to you.
