Charge-sheet for Strike

(Place) you along with other workers stopped the work of the Factory in a concerted manner thus resorting to strike without proper notice in support of the work of the demands as contained in the Charter of demands of the Union at a time when an Industrial Dispute referred by the appropriate Government to the Industrial/Labour Court by a reference Order No........... dated ........ was pending. In view of the tendency of adjudication proceedings, the strike brought about and continued is in breach of the provisions of the Industrial Disputes Act and as such is deemed illegal and makes you liable for severe disciplinary action, including dismissal from service in accordance with Standing Order No. ......... or/Service Rules.
Before we take any action, you are directed to show cause within ........ hours of the receipt of this charge-sheet as to why disciplinary action should not be taken against you for the aforesaid gross misconduct. If no reply is received by the date fixed, the management shall have the right to proceed against you in the absence of your explanation.
If you failed to give reply within time prescribed then it will be presumed that you have accepted the charge and no explanation to offer therefore action will be initiated as per Law.
