It came to notice that on ............ being working as a driver you were ordered by the Office Manager to take that on ................. being employed and working as a driver you were ordered by the Office Manager to take him in Car No............. from .............to ............where he had to go urgently for an official work. In utter violation of the instructions, you refused to take him in the car by falsely stating that there were no written orders to take the car. As a result of your failure to do so, the urgent work of the company suffered. You have, therefore, committed a breach of Order No ........ and have thus made yourself liable for disciplinary action.
You are hereby required to submit your explanation to the aforesaid charge within ...... days from the date of receipt of this charge sheet as to why suitable disciplinary action should not be initiated against you.
