It has been reported against you as under: (a) that, you are habitually remaining absent without prior permission or post-facto approval: and (b) that, in a period of.................. months i.e., from............... you have remained absent for .............days without any authorization, whatsoever.
The Charge leveled against you, if proved then disciplinary action will be initiated against you a charge sheet dated............. sent to you at residing address by registered post but same was returned back with remark “Left”.
Under the circumstances, it is considered expedient to hold an enquiry into the charge leveled against you. Accordingly you are hereby directed to be present on ..............at............... at the Industrial Relations Department for the purpose of the said enquiry Mr. .................. has been appointed to hold the enquiry as Enquiry Officer.
During the course of the enquiry you will be given full opportunity to conduct your defence by examining your witness, if any, including yourself, and cross-examining the Management's witnesses, if you so desire At the enquiry, you may take the assistance of a co-workman of the Company to conduct your defence, if you so desire. You may if you so desire, submit your explanation to this charge sheet before the commencement of enquiry proceedings.
