I have read and understood/have been read over and understood the terms of my employment and the Standing Orders and Rules of the Company as applicable to me. I have particularly noted, and I agree to the provisions relating to the rates of pay, leave facilities, payment of dearness allowance, bonus, provisions regarding discipline, payment for extra-time work and/or overtime work and the provisions relating to the stoppage, shut-down and reduction. I also understand and agree that the provisions for privilege leave collectively and individually, are very much advantageous, and therefore, I do not claim in this respect the provisions laid down under the Indian Factories Act. I also understand and agree that, although the dearness allowance by itself in comparison with other industries may not be high, but taken together with the basic salary it is much more beneficial and advantageous and as such I do agree with the scheme of dearness allowance.

I declare that the information and particulars I have given in my Proposal Form of
Employment are true.

I further declare that I have my own residential accommodation and will not ask for the same from the Company.

I also declare there is no direct relation of mine working in this factory.
