Sheweth:
1. That your petitioner was on the ......... day of ...... lawfully married to .......... and such marriage is still subsisting.

3. That such decree was obtained in the absence of and by practice of fraud on your petitioner, who was then residing at and thus kept completely ignorant of the same.
