1. I am the proprietor of M/s .....................and thus acquainted with the facts deposed to below.
2. That I am maintaining all the books of account of business being carried on by me under the name and style of M/s .......................regularly, viz. cash book, ledger, stock book, purchase vouchers, cash memo, 'C' Form register.
3. That on...................at about.................... a.m. I received the news of death of my relative Shri............. and I went to attend the funeral ceremony at ..................leaving my son Shri ........................aged about
